Setting up single LLC as disregarded entity

Hi

I am non US resident. I have formed a US based LLC in WY. I am single owner and I want my LLC to be dealt with as disregarded entity ,please advise what shall be filled in the tax information:

1) What is your tax classification? per the guidance in the page, it shall be individual

2) Are you a United States (U.S.) citizen, U.S. permanent resident (green card holder), or other U.S. resident alien? if i say No as i am non resident, it would not allow me to enter the EIN!! if I say " Yes" which is not true. it allow me to enter EIN with the following explanation (If you are a single-member LLC, enter the owner's SSN or EIN. Do not enter the disregarded entity's EIN.)

3) Address, it is not know if i shall put the business address or my address in Canada?

Please help
